Evergreen Care Center is in network with most PPO Insurance Plans and is subject to change at any time. With the constant changes in insurance plans and coverage we always recommend that you check with your insurance care provider prior to coming to our facility. If we are not in your network, the insurance company will not cover the cost of your visit. We do offer a financial hardship self pay discount if you do not have insurance at the time of your visit. We do this to help make the cost of the visit affordable for our patients. Please contact us for details.

Workman’s Comp is accepted for patients that provide all necessary documentation at the time of service. Approval must be verified through the employer before the patient is seen by our providers. For this reason we cannot accept first time worker’s comp patients after 6pm Monday thru Friday as there will not be enough time to verify employment and approval.
